The operator's recall report to the federal safety regulator says its vehicles that were stopped or stopping for a school bus with flashing red lights or an extended stop arm would proceed again before the bus had deactivated them, that the behaviour appeared with a software release broadly deployed in mid-September 2025, that it was aware of 12 citations between 25 September and 14 November 2025, and that 3,067 vehicles were potentially involved; the fix was rolled out in November.
