The FDA's own decision summary for the De Novo request DEN190040 (Caption Guidance). The summary's only date line, 27 August 2019, is when the request was received; the FDA's De Novo database record gives the decision — granted on 7 February 2020 — and that is the date used here. The software is indicated to assist medical professionals in acquiring standard views of adult transthoracic echocardiography; the pivotal study validated its use by users with no prior scanning experience to acquire limited ten-view, point-of-care echocardiograms, and the summary describes its automatic capture as emulating the way a sonographer knows an image is of sufficient quality. Its warnings leave image quality with the user and require interpretation by qualified medical professionals. It establishes what the product may be marketed to do, not how widely it is used or whether it replaces full examinations.
What this means
A regulator has accepted that software can guide people who have never scanned through a limited heart ultrasound, by telling them how to move the probe and capturing the image when it is good enough. For a sonographer that makes routine views faster to get — and opens a limited scan to nurses and other staff, whose images someone who knows ultrasound will need to check.
What it does not yet show
An authorisation says what the product may be sold to do; it does not show how widely it is used, whether it is used instead of full examinations, or any change in how many sonographers hospitals employ.
