Tasks this bears on
Deciding how it should be built
Choosing the architecture, the failure modes you are willing to have, and what the team will still be able to operate in two years.
Still human-led✓ Evidence-backed
Where this applies
The security vendor's own root cause analysis of the July 2024 update that crashed Windows machines worldwide. It finds that the automated content validator contained a logic error; that for automated testing a static set of 12 test cases was selected; and that each template instance should be deployed in a staged rollout, with two independent third-party software security vendors engaged to review further. It is the company's account of its own failure; the automation that failed was a validator, not AI.
What this means
The automated check was the weak point, and the answer was not a better check alone but a design choice — never ship everywhere at once. Deciding which failures the system is allowed to have is still a person's call.
What it does not yet show
One vendor's account of one incident; the failing automation was not AI, and it does not show how design decisions are made elsewhere.
What you can check
Open CrowdStrike's Channel File 291 Incident Root Cause Analysis and find "Each Template Instance should be deployed in a staged rollout".
Does it change the assessment?
No. The impact index is never moved by a single event. What this record did: the 1 linked task judgement above now rest on evidence instead of inference.
