Where this applies
A practice standard from the two US professional associations for clinical documentation specialists and health information coders, replacing the 2022 version. Its section on technology says technology-generated or automated queries, including prompts, nudges, alerts and queries drafted by large language models, must include all elements of compliant query practice; that organisations should not assume a vendor-supplied or EHR-integrated tool produces compliant queries by default; that humans remain responsible for ensuring queries are compliant; and that organisations using AI-assisted query delivery without pre-delivery human review should audit its output regularly. Review before delivery is a 'should', and auditing afterwards is accepted instead. It is a professional standard, not law, and it does not measure how many queries are machine-drafted.

Machines now draft the questions coders send to doctors, and the profession's own standard answers by keeping responsibility with a person: the query must meet every rule whoever drafted it, and a human answers for it — though checking each one before it goes out is recommended, not required.
