Where this applies
Ontario, Canada. The regulation says a client health record shall be kept for each client and must include particulars of all advice given by the member and particulars of the treatment applied at each visit with the name of the member who applied it, that every entry must be dated and the identity of the person who made it identifiable, and that the member shall take all reasonable steps to ensure records are kept accordingly. Read in the consolidation from 1 July 2024. It makes the therapist accountable for the record; it does not forbid software from drafting notes.
